From Circumcision to Penis Turkey Neck – Male Genital Q & A –

1. Can a droopy scrotum be lifted?
   A droopy scrotum is an appearance that many men would prefer not to have, but this issue can go beyond the cosmetic factor. It can be difficult to conceal a droopy scrotum when wearing certain types of clothing. Furthermore, certain activities – such as jogging – can lead to discomforts from chaffing.

   It is possible to lift a droopy scrotum. Aside from removing the irritation or discomfort of unwanted skin, you will create the optical illusion that your penis is longer when flaccid and erect.

   A droopy scrotum can be reduced with an incision on the back side, so any indication of surgery will not be visible from the front. Our approach is also to remove skin from the back side so the scar in not apparent face-on. The testes will rest on the elevated scrotal hammock, as this surgery is done external to the testicular tunics.

   For every inch of elevation you request, we need to remove 2 inches of skin vertically. Your testes should still rest below the emergence of your penile shaft as otherwise penetration will be encumbered.

   The operation can be done under local anesthesia and takes about an hour or so once we start. We’ll place some numbing cream on your skin so you will not feel the local.

   Be prepared to spend two post-operative days nearby, even if that means getting a room at a hotel or motel, to make follow-up easy and local. Early mobilization is the great enemy of a good cosmetic result when it comes to surgery for a droopy scrotum. It is also recommended that you bring a scrotal support or jockey briefs for post op comfort.

2. Can You Perform a Low and Tight Circumcision?
   Adult male circumcision is not a one-size fits all procedure. For many reasons, men want to results that are higher or lower, and looser or tighter. In that light, it is not uncommon to wonder if it’s possible to have a low and tight circumcision.

   While it is possible to do a low and tight circumcision, It should not be done so tightly that it draws your scrotum onto the underside of your penile shaft or pulls your pubis hair onto the top side of the penis.

   Also, if your circumcision is very low, near the “neck” of the shaft, the circumference in that area is smaller than the mid-shaft which means we will have to make a dart on the underside of your penis to make both circles fairly equal. This minimizes puckers. The dart is closed as a fine line on the underside of your penis, in line with the raphe.

   When you are seen at the office we will pre-mark your skin with a plastic surgery pen, for your approval. Additionally all of our patients have access to a handheld mirror in the operating room, so if you want it tight, we can do that.

3. Penis Turkey Neck – Does this need to be corrected?
   A penis turkey neck, also called a penile scrotal web, is a common condition that may not cause any issues at all, but that does lead some men to experience irritation during sexual intercourse.

   Some men are born with this additional skin in the transitional zone, as a result of their scrotal development. This means that while it is seen in patients who have not had any genital surgery. Still, others have it as a result of having an aggressive circumcision.

   We use a midline incision on the underside of the penis which conceals very well, as opposed to the Z-plasty technique.

   The patients are pre-marked so you can see exactly what we are doing. Essentially we use a V-Y incision which leaves the stem of the Y or vertical line on the underside of your penis. Our patients favor this approach. The pigmented line on the underside of your penis which continues onto the scrotum is called the raphe and this is where the stem of the Y winds up.
